How do I make the initial functional medicine appointment?

 Complete and submit the contact form.  You will be contacted within 48 hours during business hours to schedule your free discovery15 minute phone consult with Linda Lea, APRN-CNP.  This conversation will help us determine if this is a good fit for you and your health needs and goals.

Does your office participate with my insurance?

We do not accept or participate with any insurance plans, meaning, we are out-of-network with all insurance carriers.  For patients who have Medicare, Medicaid, and Tricare, you may not request reimbursement from these insurance companies.  You will also be asked to sign a waiver that acknowledges this government policy.

What is the cancellation policy?

Our policy is at least a 2 day notice to cancel or reschedule an initial visit. If there are missed or no-show appointments, you will receive a letter of dismissal.

What is the price/structure of appointments?

• When you are ready, complete and submit the contact form, and we will contact you to setup a free 15 minute discovery telephone appointment in which the fee structure will be discussed for your specific needs.
• Next, the adult intake form will be emailed or faxed to you. We value your time, and realize that office visits may be an interruption in an otherwise very busy schedule. For this reason, we’ve taken steps to assure that your time in our office is as focused and efficient as it can be.
• These forms are our first introduction to you, as a patient. Please complete the paperwork with thoughtfulness. Linda Lea will review all submitted intake forms prior to your scheduled, in person appointment.
• Forms must be completed and submitted to the office at least 3 days before your appointment, or sooner. Linda needs time to review your history, symptoms and past labs (labs from the past year) prior to the actual appointment.
• The initial visit can range from 1- 2 hours in length.
• The follow up visit, depending on the complexity of labs ordered can range from 30 minutes to an hour.
• Follow-up consults to review lab results are usually 30-45 minutes. Depending on the complexity of the lab results, most results could be done in person, phone/via secure video portal.
• Six week, 3 month, or 6 month follow-ups typically are 30 minute appointments.
• All fees are due at the time of service

Are you a Primary Care practice?

We are not a Primary Care practice. Keep the relationship with your Primary Care provider to continue medication management, acute illnesses and routine health appointments.

What forms of payment will be expected?

Check, cash, Mastercard, Visa, Flex spending and health savings may be an option for lab payments.

Are laboratory tests covered by my insurance?

• Patients are usually sent to a local laboratory that accepts and bills their insurance for lab testing. Most common laboratory tests are usually covered by insurance. It is your responsibility to contact your insurance carrier to ask who their preferred lab is. There is no guarantee that labs will be reimbursed by your insurance. Our office does not submit insurance billing claims.
• Wholesale self-pay prices for lab testing can be discussed at your appointment.
• Flex spending and health savings may be an option for lab payments. ??

• We now have the ability, through Functional lab testing, to discover valuable information about your body’s unique needs and preferences. This is precision medicine that personalizes a treatment plan directed by genomics and a multitude of biomarkers. We can now gain access to the cellular information that directs us to specific root causes resulting in specific targeted individualized treatment.
• This means when we obtain specific information the guesswork is reduced. The road to optimal health needs the data to develop a comprehensive whole body plan. Genetic testing, metabolic testing, hormonal testing, nutrient testing, inflammatory markers, gut health markers, detox testing, and food sensitivity assessments can all be vital and instrumental in the development of a personalized plan to regain health or maintain a healthy lifespan.

• All functional and integrative labs will be discussed in detail before ordering.

• Some specialized labs may not be covered by your insurance. You are not obligated to purchase any specialty labs. All costs and fees for any labs will always be discussed.
